The short answer is no,
The short answer is no, hemorrhoid cream is not good for puffy eyes.
First off, as a pharmacist, I would never recommend using a drug in a manner for which it is not intended. Second, there is no clinical evidence that this treatment is effective, and some risk that it might be harmful.
The urban legend that hemorrhoid cream is good for puffy eyes has been around for years. So long ago, in fact, that the cream first attributed with this beauty secret has actually been reformulated and no longer contains the ingredient that was purported to reduce puffy eyes (live yeast cell).
Now hemrrhoidal creams contain ingredients such as phenylephrine, pramoxine, or hydrocortisone. Phenylephrine is a commonly used decongestant and works by shrinking blood vessels, but there is no evidence it works for puffy eyes, and if used often, it may even lead to rebound congestion or an increase in blood flow and puffiness.
Both pramoxine and hydrocortisone reduce inflammation so you'd think they'd be good for puffy eyes, but there are other things to consider. The pramoxine could be very irritating to the eye, and then puffiness would be the least of your problems. Hydrocortisone is inadvisable because of the delicate skin around the eyes. It is different than the skin in other areas of the face and may be damaged by steroids.
Bottom line, (no pun intended) avoid hemorrhoid cream around the eyes. You'd be better served avoiding alcohol and salt, treating allergies if they are a contributing factor, getting adequate sleep, removing any eye make-up completely before bed, and drinking plenty of non-caffeinated fluids.
